Agammaglobulinemia (ARA) X-Linked Agammaglobulinemia (XLA) was first described in 1952 by Dr. Ogden Bruton. This disease, sometimes called Bruton's Agammaglobulinemia or Congenital Agammaglobulinemia, was one of the first immunodeficiency diseases to be identified.

What is the difference between Hypogammaglobulinemia and agammaglobulinemia?

"Hypogammaglobulinemia" is largely synonymous with "agammaglobulinemia". When the latter term is used (as in "X-linked agammaglobulinemia") it implies that gamma globulins are not merely reduced, but completely absent.

What causes agammaglobulinemia?

X-linked agammaglobulinemia is caused by a genetic mutation. People with the condition can't produce antibodies that fight infection. About 40% of people with the condition have a family member who has it.

What is Btk deficiency?

A few mutations in the BTK gene have been found to cause isolated growth hormone deficiency type III, a condition characterized by slow growth, short stature, and a weakened immune system. Mutations that cause this condition lead to production of a nonfunctional version of the BTK protein.

Is XLA a SCID?

XLA was also historically mistaken as Severe Combined Immunodeficiency (SCID), a much more severe immune deficiency ("Bubble boys"). A strain of laboratory mouse, XID, is used to study XLA. These mice have a mutated version of the mouse Btk gene, and exhibit a similar, yet milder, immune deficiency as in XLA.

How long do people with XLA live?

Mortality/morbidity. Most men with X-linked agammaglobulinemia (XLA) live into their 40s. The prognosis is better if treatment is started early, ideally if intravenous immunoglobulin G (IVIG) is started before the individual is aged 5 years.

What is Brutons?

Introduction. Bruton agammaglobulinemia or X-linked agammaglobulinemia (XLA) is an inherited immunodeficiency disorder characterized by the absence of mature B cells, resulting in severe antibody deficiency and recurrent infections.

Where is the BTK gene located?

The Btk gene is located on the X chromosome (Xq21. 3-q22). At least 400 mutations of the BTK gene have been identified.

Is agammaglobulinemia an autoimmune disease?

Agammaglobulinemia is a group of inherited immune deficiencies characterized by a low concentration of antibodies in the blood due to the lack of particular lymphocytes in the blood and lymph. Antibodies are proteins (immunoglobulins, (IgM), (IgG) etc) that are critical and key components of the immune system.

How common is XLA?

XLA occurs in approximately 1 in 200,000 newborns.

How do you diagnose agammaglobulinemia?

The diagnosis of agammaglobulinemia should be considered in any child with recurrent or severe bacterial infections, particularly if the patient has small or absent tonsils and lymph nodes. The first screening test should be an evaluation of serum immunoglobulins.

What are symptoms of SCID?

• failure to thrive.
• chronic diarrhea.
• frequent, often serious respiratory infections.
• oral thrush (a type of yeast infection in the mouth)
• other bacterial, viral, or fungal infections that can be serious and hard to treat, such as:

Is SCID curable?

The only cure currently and routinely available for SCID is bone marrow transplant, which provides a new immune system to the patient. Gene therapy treatment of SCID has also been successful in clinical trials, but not without complications.

What is the life expectancy of someone with Hypogammaglobulinemia?

The life expectancy of CVID patients has considerably improved over the past 30 years [5, 63], from initially 12 years to currently over 50 years [3]. Reduced survival was significantly associated with age at diagnosis, lower baseline IgG, higher IgM and fewer peripheral B cells.

Is Hypogammaglobulinemia a disability?

Hypogammaglobulinemia, a 21 letter mouthful that is as difficult to understand as to pronounce. On the surface, it is a rare immune disorder marked by a reduction in gamma globulins (serum proteins).

Can I donate blood if I have Hypogammaglobulinemia?

We commend you for being a donor, and want you to know that yes, in most cases you can still be a donor after being diagnosed with PI. Many people think that certain medical conditions can disqualify them from donating but this is not always true.

Why do people with agammaglobulinemia have more trouble fighting bacterial infections than viral infections?

In X-linked agammaglobulinemia, there is a failure of pre-B-lymphocytes to mature into B-lymphocytes (mature B-lymphocytes produce antibodies). As a result, there are no antibodies produced, and the child's body is unable to fight off bacterial infections and some viral infections.

Is immunodeficiency the same as immunocompromised?

When your immune system fails to respond adequately to infection, it's called an immunodeficiency, and you may be immunocompromised. People may also suffer from the opposite condition, an overactive immune system that attacks healthy cells as though they were foreign bodies, and that is called an autoimmune response.

What is Hyper IgM Syndrome?

Hyper IgM syndromes are a group of rare disorders in which the immune system does not function properly. They are classified as rare primary immunodeficiency disorders, which are a group of disorders characterized by irregularities in the cell development and/or cell maturation process of the immune system.

What drugs are BTK inhibitors?

• Imbruvica (ibrutinib) Capsules and Tablets.
• Calquence (acalabrutinib) Capsules.
• Brukinsa (zanubrutinib) Capsules.

What chromosome is the BTK gene on?

BTK is critical to the maturation of pre–B cells to differentiating mature B cells. The BTK gene defect has been mapped to the long arm of the X chromosome at band Xq21. 3 to Xq22, spanning 37.5kb with 19 exons forming 659 amino acids to complete the BTK cytosolic tyrosine kinase.

Why is BTK important?

Bruton's tyrosine kinase (BTK) is a non-receptor kinase that plays a crucial role in oncogenic signaling that is critical for proliferation and survival of leukemic cells in many B cell malignancies.

What causes XLA?

Frequently called Bruton's Agammaglobulinemia, XLA is caused by a genetic mistake in a gene called Bruton's tyrosine kinase (BTK), which prevents B cells from developing normally. B cells are responsible for producing the antibodies that the immune system relies on to fight off infection.

Is XLA an autoimmune disease?

Although patients with XLA are generally considered to have a low risk of autoimmune or inflammatory disease compared to other PIDD cohorts, data from this patient survey and a national registry indicate that a significant proportion of patients with XLA have symptoms that are consistent with a diagnosis of arthritis, ...

What does Hypogammaglobulinemia mean?

Hypogammaglobulinemia refers to a laboratory finding (low immunoglobulin G, or IgG) that may be asymptomatic if mild or may be associated with a number of clinical entities with varied causes and manifestations if more extreme.
